On behalf of the SRC, I inform all students with results and portal issues to channel their

complains to the SRC through the google forms below. The SRC would forward such data to

management for redress.

Please take note of the following:

1.-You are to read instructions carefully on the forms before filling out,

2. The SRC would not be liable for any falsehood provided by any student on the google forms.
3. Any student who provides wrong information would be severely sanctioned by management including dismissal and suspensions.
4. The SRC would not bear the consequences of your actions and inactions of given false information.

1. NAME OF STUDENT(as on your portal)  
2. INDEX NUMBER(as on your portal)  *

3. DEPARTMENT (Give accurate information) *

4. LEVEL (Taken last academic year base line) *

5. course(s) with IC due to late course registration(eg. management, psychology, calculus etc)

6. course(s) which has not reflected on your portal 

7. course(s) which your grade has changed when loaded onto portal ( course name, original grade,  portal grade. eg. accounting  B+ , portal grade C)

8. course(s) with exceptional issues( state the issue. eg. accounting, I did not take part in mid semester and so I have an I..C etc.)
